Additional documents will be requested via email or phone call upon completion of the online application form. Please be prepared to provide the following when required:
- Child’s Birth Certificate
- Child’s Current Physical Exam/Immunization Records, signed by their Physician (must be within the last 13 months)
- Any additional medical records you think we may need (Asthma, Food Allergies, etc.)
- Proof of Residency (MUST have these 3 items)
- Valid Driver’s License or Photo ID (Passports are also acceptable)
- Copy of utility bill (Electric, Heat, Cable, Water etc.) within the last 60 days
- ONE of the following: Recent mortgage bill, copy of lease/rental agreement, or letter from Housing Office
